What is Conozca Su Corazón?

Conozca Su Corazón is a community based wellness program aimed at reducing the devastation of cardiovascular diseases in the Latino community.  Conozca Su Corazón contains important education about risk factors, vital numbers (cholesterol, blood pressure & glucose), heart healthy eating habits and the benefits of living a more active lifestyle.  This program also provides health screenings to participants in order to track their vital numbers throughout the program. Conozca Su Corazón aims to increase knowledge and awareness of cardiovascular disease and stroke as well as to provide participants with relevant and practical ways to improve health.

 

How does Conozca Su Corazon benefit the community?

Heart disease and stroke rank as the number one and number three killers of Latinos. They claim the lives of 30 percent or more than 107,000 Hispanics each year.  Although these numbers paint a disturbing picture, there are things that can be done now to improve health among Latinos. The biggest asset that we have in the fight against heart disease and stroke is knowledge. We encourage you to “Learn and Live” by implementing Conozca Su Corazón and turn knowledge into a benefit to improve the health of the Latino community in a meaningful and memorable way.

 

Who should implement Conozca Su Corazón?

Any organization that has consistent membership and established meetings with a group of individuals at a designated location is the best candidate for Conozca.  This program is best implemented when the participants meet with some level of consistency (PTOs, seniors groups, parent groups, church groups).

 

How long does the program take to complete?

Conozca Su Corazón can be implemented in a several different formats. The “Workshop” curriculum is ideally suited for small congregations and small organizations who are interested in a simple and quick one day program or awareness day.  The “Activity” curriculum is suited those who would like to implement three different health activities over the course of any given time frame up to six months. The curriculum for the program focuses on heart disease, stroke, nutrition and physical activity.

 

Conozca Su Corazón Challenge” curriculum is for organizations that committee six months continuous health education. Performance is evaluated by point totals for minimum requirements and bonus point activities. Organizations will be ranked by highest point total. The following are the scholarship awards:

1st Place             $2,000

2ndPlace             $1,000

What resources are available during the program?

Health screenings are provided at the beginning and end of the programs implementation.  In addition, incentive items relevant to the program modules are provided to the participants. We have various community partners available to provide cooking demonstrations, nutrition classes, fitness demonstrations and walking clubs. We also work with other organization to help educate participants about various health topics such as breast cancer, prostate cancer, diabetes and smoking. All of these resources are FREE to participating organizations.
